1.  COVID-19 SCREENING & CONSENT FORM

Currently, this will need to be completed and returned via email (if possible) within 48 hours of your appointment.

In order to complete the form electronically, after downloading, go to File Explorer to Open in Adobe Acrobat.    On the right side you should see 'Fill & Sign'.  Click on this to enable you to fill in the form.  Save your responses and email the file to me.

 

Alternatively, you may submit a form directly via the website.

2.  COVID-19 RISK ASSESSMENT

This is my up-to-date risk assessment and will be reviewed regularly as government guidelines and advice from the Sports Massage Association changes.
